29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. a. On or about October 27, 2025, the employer did not ensure that a wooden pallet used as a scaffold platform was attached to the Mitsubishi forklift (Model: FG 30 S/N: AF13D-32018) and permitted the forklift to be moved horizontally while the platform was occupied. As a result, the employee was exposed to fall hazards greater than 14 feet.

29 CFR 1910.27(a): Scaffolds used in general industry must meet the requirements in 29 CFR part 1926, subpart L (Scaffolds). 29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use. a. On or about October 27, 2025, employees used a Mitsubishi forklift (Model: FG 30, Serial: AF13D-32018) to elevate a wooden pallet used as a job-made scaffold platform. The employer failed to ensure the platform was specifically designed by the equipment manufacturer which exposed an employee to fall hazards.
