29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about 11/21/18, the employer failed to ensure that the following steps were taken when employees are changing the belts on the Glaston Continous Line: 1) Prepare for shutdown, 2) Machine or equipment shutdown, 3) Machine or equipment isolation, 4) Lockout/tagout device application, 5) Release/relieve stored energy, and 6) Verification of isolation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 11/21/18, the employer failed to ensure that the guards on the Glaston Continuous Tempering Line were secured. The guards covering the Glaston Continuous Tempering Belts could be lifted while the machine was in operation.
