29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about March 1, 2024, Intigral Inc. located at 7850 Northfield Road Walton Hills Ohio 44146, did not ensure machine specific procedures for the CT Quench (Glaston Corp.T, Tampglass 48' CHF, Project 8008) were utilized prior to service and maintenance of the equipment took place. Operators tasked with operating the CT Quench removed guarding in place to reattach the rubber band belts on the continuous line pulleys, while in operation exposing the operators to the hazardous area.
