29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee was protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems, as required by sub-items A through D of this section: a) On or about November 21, 2018 - Roof, employees were exposed to fall hazards of approximately 25 feet when they walked on the low slope roof to paint fans/vents, walked on the roof to check fans/vents, and walked on the roof to check the light near the wind sock without fall protection.

29 CFR 1910.30(a)(1): Before any employee was exposed to a fall hazard, the employer did not provide training for each employee who used personal fall protection systems or who was required to be trained as specified elsewhere in this subpart: or in the alternative 29 CFR 1910.30(c): The employer did not retrain an employee when the employer had reason to believe the employee did not have the understanding and skill required by paragraphs (a) and (b) of this section. Situations requiring retraining include, but are not limited to, the following: When changes in the workplace render previous training obsolete or inadequate; when changes in the types of fall protection systems or equipment to be used render previous training obsolete or inadequate; or when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee no longer has the requisite understanding or skill necessary to use equipment or perform the job safely. a) On or about November 29, 2018 - Roof, employees were exposed to fall hazards when they had not been trained to know where to find an anchor for a harness/lanyard on the roof, had not been trained to stay 15 feet from all fall hazards if not using fall protection, had not been trained to use a guardrail system, safety net system, travel restraint system, or a personal fall arrest system (or a designated area when performing work that is both infrequent and temporary) between 6 and 15 feet from fall hazards, and had not been trained to use a guardrail system, safety net system, travel restraint system, or personal fall arrest system if less than 6 feet from a fall hazard.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent: a) On or about November 29, 2018 - End of building where bay 1 and bay 2 are located, employees were exposed to fire hazards when there were no signs to indicate that the path to exit in the steel coil storage area was past racks into a chain link cage around materials on the floor to the side hinged door marked as an exit.
