29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval: a) The employer utilized boom attachments in buildings #4 and #5 to move tank rings and components without the forklift manufacturer's approval and without the weight of the attachment available to ensure employees did not overload the forklift. b) The employer utilized plate basket attachments in buildings #4 and #5 to move tank sections without the forklift manufacturer's approval and without the weight of the attachment available to ensure employees did not overload the forklift. Imperial Industries Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(a)(4), which was contained in OSHA inspection number 1007781, citation number one, item number two, and was affirmed as a final order on March 31, 2016, with respect to a workplace located at 505 Industrial Park Ave. Rothschild, WI 54474.
