29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Sawmill Grading Station ; On or about November 28, 2018 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to rotate by hand the pulley and belt to the gearbox driving the floor waste chains in order to unjam the chain without first affixing a lockout device on the machines isolating device to secure the machine from the energy source. b) Sawmill Chipper; On or about December 4, 2018 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to work on the Chipper to replace blades without first affixing a lockout device on the machines isolating device to secure the machine from the energy source. c) Sawmill Edger Saw; On or about December 4, 2018 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to work on the Edger Saw to replace blades without first affixing a lockout device on the machines isolating device to secure the machine from the energy source. Lassiter Lumber, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 1126559, citation number 1, item number 1 and was affirmed as a final order on April 28, 2016, with respect to a workplace located at 107 Milry Mill Road, Milry AL 36558.

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid: a) 1472 Hwy 17, Silas AL; On or about December 4, 2018 and at times prior thereto employees were exposed to potential medical emergencies in that emergency treatment services were not within reasonable proximity of the worksite and the employer did not have a trained first aid provider at the worksite at all times when employees were working.
