29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: Or in the alternative Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a) On or about November 7th, 2018, and at times prior, for the employees who were exposed to a fall hazard of up to 10 feet while framing a residential structure from a 20 foot long man basket that was attached to a telehandler that exceeded two times load center distance on a JCB 512 -56 rough terrain forklift, Serial # JCB5CHXGE0143291, in the vicinity of 3259 Oak Ridge Loop E, in West Fargo, North Dakota.
