29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii): a) Printer #101 - The operator (Printer) easily accessed the central impression cylinder during operation, becoming exposed to ingoing nip points from drive gears and caught in hazards from a large drive gear without a face cover, on or about December 10th, 2018. b) Printer #104 - The operator (Printer) easily accessed the central impression cylinder during operation, becoming exposed to ingoing nip points from drive gears and caught in hazards from a large drive gear without a face cover , on or about December 10th, 2018. c) Printer #105 - The operator (Printer) easily accessed the central impression cylinder during operation, becoming exposed to ingoing nip points from drive gears and caught in hazards from a large drive gear without a face cover, on or about December 10th, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
