The ladder the employees were using to access the second story roof was not extended three feet beyond the landing or edge of the roof.

The crew lead and two employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. Lo Maximo Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1229441, citation number 1, item number 1, issued on 9/06/2017, and was affirmed as a final order on 9/15/2017 with respect to a workplace located at 508 Tweed Drive in Saint Louis, MO.
