29 CFR  1926.503(a)(1): The employer had not provided a training program for each employee who might have been exposed to fall hazards. The program had not enabled each employee to recognize the hazards of falling and had not trained each employee in the procedures to be followed in order to minimize these hazards.  On or about October 31, 2024, Lo Maximo Construction LLC did not provide training for each employee who was exposed to fall hazards while roofing a residential building at 143 Shadow Pointe Drive in Wentzville, Missouri. The employees were not able to recognize fall hazards and were not trained in the procedures to be followed to minimize fall hazards.

29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On or about October 31, 2024, Lo Maximo Construction LLC did not ensure that a portable extension ladder, which was used to access the roof of a residential building at 143 Shadow Pointe Drive in Wentzville, Missouri, was extended at least 3 feet above the upper landing surface.

29 CFR  1926.102(a)(1): The employer had not ensured that each affected employee had used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about October 31, 2024, Lo Maximo Construction LLC did not ensure that each employee operating a pneumatic nail gun used appropriate eye protection when exposed to eye hazards from flying particles in a worksite at 143 Shadow Pointe Drive in Wentzville, Missouri.  Lo Maximo Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1649279, citation number 1, item number 1 and was affirmed as a final order on March 29, 2023, with respect to a workplace located at Kennedy Lane, Lot 46 in Manchester, Missouri.

On or about October 31, 2024, Lo Maximo Construction LLC did not ensure that each employee roofing a one-story tall residential building, at 143 Shadow Pointe Drive in Wentzville, Missouri, utilized a personal fall arrest system.  Lo Maximo Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1649279, citation number 1, item number 2 and was affirmed as a final order on March 29, 2023, with respect to a workplace located at Kennedy Lane, Lot 46 in Manchester, Missouri.

The chemical gasoline was used by employees to power air compressors for pneumatic nail guns in a worksite at 143 Shadow Pointe Drive in Wentzville, Missouri.
