29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Structural Shop: On or about December 11, 2018, and at times prior, workers were exposed to laceration hazards while operating a Hyd-Mech S-23 horizontal band saw while the blade guard was not installed around the unused portion of the blade between the head and blade guide. b) Parts cleaning area: On or about December 11, 2018, and at times prior, workers were exposed to caught-in hazards at a shop-made belt sander that was not fitted with a cover to prevent access to the ingoing nip point of the lower belt driver. 29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Assembly: On or about December 11, 2018, and at times prior, workers were exposed to potential hazards while operating a Rikon bench grinder while the tongue guard of the right abrasive wheel was adjusted to 5/16 inch. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet). Diversified Machine Systems, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.215(b)(9), which was contained in OSHA inspection number 1160052, citation number 1, item number 001 and was affirmed as a final order on August 4, 2016, with respect to a workplace located at 1068 Elkton Dr., Colorado Springs, CO 80907.
