29 CFR  1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments. The employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  (a) Diversified Machine Systems, LLC at 1068 Elkton Dr. Colorado Springs, CO 80907: On or before 10/19/21 the employer did not protect employees from injury in that forklifts were using job-made fork extenders constructed out of tube steel. The extenders lacked a method of being secured to the mast and were of a slip-on design. These conditions exposed employees to hazards including but not limited to struck-by, crushing, and tip-over. 29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Diversified Machine Systems, LLC at 1068 Elkton Dr. Colorado Springs, CO 80907: On or before 10/19/21 the employer did not protect employees from injury in that the Hyd-Mech S20 horizontal bandsaw lacked guarding on the non working portion of the blade opposite the end with the moving blade guide. This condition exposed employees to laceration and amputation hazards.  (b) Diversified Machine Systems, LLC at 1068 Elkton Dr. Colorado Springs, CO 80907: On or before 10/19/21 the employer did not protect employees from injury in that the Diversified Machine Systems "Mini Planer" lacked adequate guarding. A fabric and vinyl curtain was used to block chips. This condition exposed employees to struck by, laceration, and flying chips.   Abatement Note: Abatement certification is required for this item. 29 CFR  1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  (a) Diversified Machine Systems, LLC at 1068 Elkton Dr. Colorado Springs, CO 80907: On or before 10/19/21 the employer did not protect employees from injury in that the H1E and L1F panels in the inventory area had items stored within 30 inches of the panels. This condition exposed employees to arc flash and fire hazards.  Abatement Note: Abatement certification is not required for this item.
