29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance: a. At the establishment, lock-out/tag-out procedures for the Alvey Diverter were incomplete in that the procedures did not designate the sequence in which the specific machine lock-out/tag-out elements were to be performed. Machine specific lock-out/tag-out procedures must be performed in the sequence as described in 1910.147(d)(1) through 1910.147(d)(6). b. At the establishment, lock-out/tag-out procedures for the Alvey Diverter were incomplete in that the procedures were not specific and in the detail to describe the actions employees were to take during each of the required steps, 1910.147(d)(1) through 1910.147(d)(6) to successfully lock-out/tag-out the machine.
