29 CFR  1910.119(f)(1)(i)(D):The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in emergency shutdown including the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown was executed in a safe and timely manner.  (a) On or about October 27, 2020 and at times prior thereto, at the Anheuser-Busch Houston Brewery, the employer exposed workers to ammonia chemical hazards when it did not develop and implement written procedures that provide clear instruction for safe emergency isolation of the #3 High Pressure Receiver.

29 CFR  1910.119(f)(3):The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities.   (a) On or about October 27, 2020 and at times prior thereto, at the Anheuser-Busch Houston Brewery, the employer exposed workers to ammonia chemical hazards when it  did not review the operating procedure for emergency shutdown of equipment  often enough to ensure that it was current and accurate for use to .isolate the #3 High Pressure Receiver.  (b)  On or about October 27, 2020 and at times prior thereto, at the Anheuser-Busch Houston Brewery, the employer exposed workers to ammonia chemical hazards when it  did not review the operating procedure for pumpout of receiver vessels  often enough to ensure that it was current and accurate for pumpout of the  #3 High Pressure Receiver.

29 CFR  1910.119(j)(5):The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) in a safe and timely manner when necessary means where taken to assure safe operation.    (a)On or about October 27, 2020 in the Powerhouse of the Anheuser-Busch Houston Brewery the employer did not correct deficiencies of the inlet valve to the #3 High Pressure Receiver in a timely manner.   (b) On or about October 27, 2020 in the EVAP Deck area on the roof of the Powerhouse of the Anheuser-Busch Houston Brewery the employer did not correct deficiencies in the ammonia piping down comer, piping going to the pump out system and other critical piping to the receivers , in a timely manner.   (c) On or about October 27, 2020 in the EVAP Deck area on the roof of the Powerhouse of the Anheuser-Busch Houston Brewery the employer did not correct deficiencies in electrical and controls system, in that there were broken and open conduit with electrical wires visible inside for the ammonia system in a timely manner.

29 CFR  1910.212(a)(5):Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:  (a) On or about December 11, 2020 and at subsequent times in the EVAP Condenser area on the roof of the Power House at the Anheuser-Busch Houston Brewery, a motor in the circulating water system for the condensers had openings larger than one-half inch in the motor fan guard, exposing workers to hazard of coming into contact with the fan blades.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems; safety net systems; or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.  On or about October 27, 2020 in the Power House at the Anheuser-Busch Houston Brewery, employees who work from the work platform above the high pressure receiver vessels were exposed to falls from elevated heights greater than 4 feet; when the employer failed to ensure sections of the guard rail system that were missing was repaired and  or replaced.

29 CFR  1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors enter were not effectively closed.   (a) On or around October 27, 2020 and at times subsequent thereto, in the Evaporative Condenser area on the roof of the Power House of the Anheuser-Busch Houston Brewery, the employer exposed workers to electric shock hazards and possible exposure to hazards associated with ammonia when conductors entering fittings in the control system of the Evaporative Condensers were not protected from abrasion where openings through which the conductors enter were not effectively closed.
