29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 12/17/18, the employer failed to ensure that the Divine Impact Grinder, Serial #22055, was protected by machine guarding methods: a) The barrier guard on the right station had an opening about 2" in width and about 11" in height.
