29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about July 25, 2025, the employer did not ensure that machine specific procedures were developed and utilized for the control of hazardous energies on the media blasting cabinet. Employees were tasked with service and maintenance of the blasting cabinet including air filtration device changes. Hazardous energies present on the the equipment include but are not limited to pneumatic and electrical.

29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about July 25, 2025, the employer did not ensure that a periodic (annual) inspection of the energy control procedures for equipment had taken place at the facility. Equipment present at the facility includes but is not limited to media blasters. Hazardous energies present on equipment includes but is not limited to pneumatic,  and electrical. A periodic inspection had not been conducted at the facility.

29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  On or about July 25, 2025, the employer did not ensure that employees tasked with the service and maintenance of equipment in the facility had received training as authorized individuals for the isolation of hazardous energies (LO/TO). Equipment present at the facility includes but is not limited to media blasters. Hazardous energies present on equipment includes but is not limited to pneumatic and electrical. Employees were tasked with the changing of the air filter on a media blaster without having been trained as authorized. The task required the control of hazardous energies.
