On 12/15/18 an employee was clearing a jam from a Little Wonder leaf vacuum and did not ensure that the engine was turned off and locked out prior to reaching inside the machine.  As a result, the employee suffered multiple fractures and lacerations to their right arm and hand, including a surgical finger amputation.

On 12/15/2018 an employee clearing a jammed leaf vacuum machine, did not take steps to isolate the energy sources prior to reaching inside the machine.  As a result, the employee suffered multiple arm and hand fractures, lacerations and a surgical finger amputation.
