29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval.   At the worksite located at 112 Missouri 291, Lee's Summit, Missouri:  Employees were observed working from a wooden box attached to the forks of the JCB Material Handler.  The company built the wooden box to work from at heights.  Modifications and additions were performed without manufacturers prior written approval as required.

29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  At the worksite located at 112 Missouri 291, Lee's Summit, Missouri:  Employees engaged in roofing activities were exposed to fall hazards of greater than 10 feet from the lowest eave to the ground while installing roofing materials without the use of fall protection systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   At the worksite located at 112 Missouri 291, Lee's Summit, Missouri:  Employees were accessing a working surface on a pitched roof by way of a portable ladder whose side rails did not extend 36 inches above the upper landing surface being accessed.
