At the worksite located at 112 Missouri 291, Lee's Summit, Missouri: Employees were observed working from a wooden box attached to the forks of the JCB Material Handler. The company built the wooden box to work from at heights. Modifications and additions were performed without manufacturers prior written approval as required.
