29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  a) Worksite, 1300 Roanoke Ave, Riverhead, NY 11901 - An employee was doing window work on the third tier of a fabricated frame scaffold. There was only one aluminum-ply plank approximately 12in. wide. One plank 6in. wide was missing. The employee did not have a fully planked working surface. The employee was exposed to falling approximately 18ft. to the roof of the first level of the commercial building, on or about 01/15/2019.   b) Worksite, 1300 Roanoke Ave, Riverhead, NY 11901 - Employees were doing window work on the first tier of a fabricated frame scaffold. There were only two planks approximately 6in. wide. One plank approximately 6in. wide was missing. One plank 6in. wide was missing. The employees did not have a fully planked working surface. The employees were exposed to falling approximately 7ft. to the roof of the first level of the commercial building, on or about 01/15/2019.   Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a) Worksite, 1300 Roanoke Ave, Riverhead, NY - Employees were doing window work from the first tier of the fabricated frame scaffold, approximately 7ft. above the roof of the first level of the commercial building and did not have a ladder or other means of safe access; on or about 01/15/2019.   b) Worksite, 1300 Roanoke Ave, Riverhead, NY - An employee was doing window work from the third tier of the fabricated frame scaffold, approximately 18ft. above the roof of the first level of the commercial building and did not have a ladder or other means of safe access; on or about 01/15/2019. The employee did not have a guardrail system, safety net system, personal fall arrest system, or any other type of fall protection; on or about 01/15/2019. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Worksite, 1300 Roanoke Ave, Riverhead, NY - Employees were using a WERNER fiberglass extension ladder to access the working level at approximately 16ft. above the ground below. The ladder extended approximately 14 inches above the landing surface.
