29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Worksite: 50 Hubbard Lane Units #14 and #15, Southampton, New York 11968 - Employees were not wearing protective helmets and were exposed to being struck by a reciprocating saw resting on a ladder rung overhead approximately 15 feet high while walking and working below overhead gutter repair work; on or about 6/4/25.  Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR  1926.451(g)(1)(i):  Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system.   a) Worksite: 50 Hubbard Lane Units #14 and #15, Southampton, New York 11968 - Employee were not protected from falling while working from a ladder jack scaffold approximately 11.5 feet above the ground below.  The employees were removing gutters on a residential building; on or about 6/3/25.  Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
