29 CFR 1910.67(c)(2)(ii): An employee(s) operating an aerial lift(s) was not properly trained:    The employer had not provided adequate training to the operators to safely operate these lift machines.    a) Employees were exposed to struck-by hazards.  Employees engaged in welding activities using the Skyjack Boom Lift, model number SJ45T, were exposed to struck-by hazards in that comprehensive Boom Lift training was not provided prior to permitting Boom Lift operation. The classroom setting formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material) was not provided to employees prior to operating the Boom Lift.    b) Employees were exposed to struck-by hazards.  Employees engaged in material handing activities using the Genie Lift telehandler, model number GTH1544, were exposed to struck-by hazards in that comprehensive Genie Lift training was not provided prior to permitting Genie Lift operation. The classroom setting formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material) was not provided to employees prior to operating the Genie Lift.    29 CFR 1903.19 (d)(1) requires certification and documentation that the abatement of the above violation is complete.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:    The employer had not provided adequate training to the operators to safely operate these lift machines.    a) Employees were exposed to struck-by hazards.  Employees engaged in material handing activities using the Doosan forklift, model number G30N-7, serial number FGA15-1290-00645, were exposed to struck-by hazards in that comprehensive forklift training was not provided prior to permitting forklift operation. The classroom setting formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material) was not provided to employees prior to operating the forklift.    b) Employees were exposed to struck-by hazards.  Employees engaged in material handing activities using the Grove RT 635C Mobile Crane were exposed to struck-by hazards in that comprehensive Mobile Crane training was not provided prior to permitting Mobile Crane operation. The classroom setting formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material) was not provided to employees prior to operating the Mobile Crane.    29 CFR 1903.19 (d)(1) requires certification and documentation that the abatement of the above violation is complete.

Employees engaged in servicing and maintenance activities at the Hypertherm Plasma Cutter Machine, serial number 2556, were exposed to electrical, amputation, caught-in, and struck-by hazards in that machine specific energy control procedures had not been developed, documented, and utilized. Sources of hazard energy include: electrical, pneumatic, and gravity.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

Employees engaged in tanks welding and grinding activities at the Top & Bottom Station in the shop were exposed to tripping hazards in that multiple flexible cords used to supply power to the DeWalt 4-1/2" Paddle Switch Small Angle Grinders were laying around in the aisle ways, and on the floor.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
