This was most recently documented on January 28, 2019 at 4457 G. Street Omaha, Nebraska. The employer failed to ensure that the chain feed mechanism on the Text Wrap machine located in the Blue Packing area was equipped with a guard to prevent an employee from reaching into this area and coming in contact with the ingoing nip point of the feed chain.
