29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about February 6, 2024, employees engaged in commercial baking and manufacturing activities, located at 4651 F St., Omaha, NE 68117, were exposed to amputation hazards in that the employees were preforming maintenance activities on machines that did not have developed lockout/tagout procedures. Instance include but are not limited to: i. Schubert baking machine installed over a year ago using the operator's manual in lieu of a written procedure. ii. Slicing Machine installed over a year ago without having developed a written procedure.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about February 6, 2024, employees engaged in commercial baking and manufacturing activities, located at 4651 F St., Omaha, NE 68117, were exposed to amputation hazards in that the employees were preforming maintenance activities on the Guillotine machine that had not undergone a periodic annual lockout/tagout procedure assessment.
