(b) Dock area: On or prior to 2/01/19, Hyster model 40 forklift no. 46 had a propane leak at a connection to the fuel cylinder, creating a potential fire hazard during operation. Land Air Express of New England was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(p)(1), which was contained in OSHA inspection number 1056816, citation number 1, item number 2a and was affirmed as a final order on 5/08/15, with respect to a workplace located at 345 Rocus Street, Springfield, MA 01104.
