29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Northeast corner of Interstate 690 and Lodi Street, easterly side of frack tank, on or about 2-6-19: Two employees used a portable fiberglass extension ladder that was at an angle in excess of one quarter (4:1 ratio) of the working length of the ladder, or the distance between the foot and the ladder support, exposing the employee to a fall hazard of 9 feet to the ground below.
