29 CFR 1910.106(e)(2)(ii): "Containers." Flammable liquids shall be stored in tanks or closed containers.    On or about February 11, 2019, the flammable liquid Xylol was not stored in a closed container.  The employer had a five gallon container of Xylol that had a shop towel stuffed in the bung hole sitting in the Paint Department at the North end of the Welding Shop.

29 CFR 1910.107(b)(10): Illumination. When spraying areas are illuminated through glass panels or other transparent materials, only fixed lighting units shall be used as a source of illumination. Panels shall effectively isolate the spraying area from the area in which the lighting unit is located, and shall be of a noncombustible material of such a nature or so protected that breakage will be unlikely. Panels shall be so arranged that normal accumulations of residue on the exposed surface of the panel will not be raised to a dangerous temperature by radiation or conduction from the source of illumination.    On or about February 11, 2019, the spray area in the Paint Department did not have panels separating the spraying area from the lighting units.

29 CFR 1910.134(g)(1)(i)(A): The employer shall not permit respirators with tight-fitting facepieces to be worn by employees who have facial hair that comes between the sealing surface of the facepiece and the face or that interferes with valve function.  On or about March 19, 2019, the employer did not ensure that the employee wearing a full-face respirator did not have facial hair that came between the sealing surface and their face.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.    On or about February 11, 2019, the power cord on the electric pedestal fan at the South end of the Paint Department had not been visually inspected before the shift.  The power cord was visibly damaged and missing the ground pin which exposed employees to electric shock.

29 CFR 1910.1200(f)(6)(ii): Workplace labeling. Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.    On or about February 11, 2019, the employer did not ensure that two disposable plastic bottles of chemicals used to lubricate the drills in the Fabrication Shop were labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof.  One of the bottles was labeled "DRILL ONLY " and the other bottle had no markings on it.  The bottles contained chemicals which the employees would use to lubricate the drill bits.
