29 CFR 1918.32(b): The edge of a hatch section, or of stowed cargo, was not guarded by a vertical safety net or other means providing equal fall protection to exposed employees: Freezer Hatch, Port Side: On or about February 6, 2019, during marine cargo handling operations an employee was exposed to fall hazard within the non-protected side of the guard rail.

29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: Engine Space, Port Side Entry: On or about February 6, 2019 and at times prior thereto, an oil pot was allowed to leak on the raised deck. This condition exposed employees to slip hazards.
