29 CFR  1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:  F/T Unimak - Engine Room - On or About February 25, 2026, employees performing maintenance activities were exposed to health hazards in that the employer had not provided an annual fit test for employee(s) required to use respirators that was current at the time of the inspection.   OSHA Instruction: Shipyard Employment "Tool Bag" Directive - CPL 02-00-162 allows for the applicability of 29 CFR 1910.134(f)(2) in the ship building and ship repair industry.

29 CFR  1910.134(h)(2)(i):  All respirators shall be stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and they shall be packed or stored to prevent deformation of the facepiece and exhalation valve.  F/V Unimak-Factory Machine Shop : On or about February 25, 2026, one Half-Face respirators was not properly stored and cleaned when not in use. This condition exposed the employees using the respirator to health hazards.

29 CFR  1910.134(k): The employer did not provide comprehensive, understandable training which did not occur annually and/or more often if necessary:  F/T Unimak - Engine Room - On or About February 25, 2026, employees performing maintenance activities were exposed to health hazards in that the employer had not provide annual respiratory protection training.   OSHA Instruction: Shipyard Employment "Tool Bag" Directive - CPL 02-00-162 allows for the applicability of 29 CFR 1910.134(k) in the ship building and ship repair industry.
