29 CFR 1910.303(b)(1)(i) as required by 29 CFR 1960.8(b): Electrical equipment was not free from recognized hazards likely to cause death or serious physical harm, in that the employer did not ensure that the electrical equipment had listing or labeling identifying the purpose for which it was designed: On or about February 14, 2019, in the tower housing the antennas, the employer did not ensure electrical equipment was used as listed or labeled in that a locally manufactured extension cord fitted with a 4 inch square electrical box with conduit knockouts was being used to power work lights. The electrical box was not designed for this purpose and is intended for use in permanent installations.

29 CFR 1910.1200(f)(6) as required by 29 CFR 1960.8(b): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section: On or about February 14, 2019, in the engine generator room, the employer did not ensure that containers storing hazardous chemicals such as but not limited to antifreeze which is used in the generator at the facility were labeled or marked with signal words, hazard statements, pictograms, precautionary statements and name, address, and telephone number of the chemical manufacturer in that a plastic 5 gallon container and a plastic spray bottle containing antifreeze did not have labeling with all required elements.
