29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  Body shop, robotic welding cells - on or about February 26, 2019, "adjunct" employees assigned to clean welding robots and their cells were not trained in the hydraulic, pneumatic, and gravitational energy sources specific to the robotic cell where they were performing work.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Location: robotic welders testing cells area, shop bench/work bench. Equipment: 12-outlets, metal casing relocatable power tap - on or about February 26, 2019, employees were exposed to fire and electrical hazards where an energized relocatable power tap was used in an industrial setting as a substitute for fixed wiring of electronic devices. Relocatable power tap was not directly connected to a permanently installed branch-circuit receptacle outlet and was connected in-series to an extension cord. Relocatable power tap was affixed with screws to a work bench. This equipment is not intended for such use.  There is no abatement certification or documentation required for this item.   (b) Location: North body shop, zone N (Sta. 12 thru 16), safety gate S14 PD1 I2.b10 I2.b11. Equipment: APC SurgeArrest relocatable power tap - on or about February 26, 2019, employees were exposed to fire and electrical hazards where an energized relocatable power tap was used in an industrial setting as a substitute for fixed wiring of a television. Relocatable power tap was not directly connected to a permanently installed branch-circuit receptacle outlet and was connected in-series to an extension cord. Relocatable power tap was zip-tied to a fence. This equipment is not intended for such use.  There is no abatement certification or documentation required for this item.
