29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about February 28, 2019, the employer failed to develop and implement machine specific procedures for locking and tagging out machines to include but not limited to brake and punch presses and roll machines.  Machines that are powered by energy sources such as electrical, hydraulic and pneumatic energy.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    On or about February 28, 2019, the employer failed to conduct a periodic inspection, at least annually, of their Lock Out Tag Out Program to ensure that it was up to date and effective.  The last documented review of this program was in December of 2014.  Lock Out and Tag Out are being performed on various machines, including but not limited to the QH 1030 press brake and the Cincinnati shearer press.

A yellow Clark vehicle, serial #356-815-4376, that was being operated had a light out and also had an unsecured propane tank.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  On or about February 28, 2019, the employer failed to ensure employees where protected from stuck by and caught in between hazards when using bench grinders, to include but not limited to the Baldor bench grinder, serial #F1085.  The distance between the work rest and the grinding wheel measured at 1.5 inches, which is greater than the allowable 1/8 inch.
