On or about February 22, 2019 the employer failed to ensure that the anchorage point for use as part of the personal fall arrest system (PFAS) to protect employees from falling into the G1 Grinding granulator was capable of supporting 5000 pounds of downward force. Employees utilized a PFAS in which the anchorage point was a metal constructed guardrail system.
