29 CFR 1910.140(c)(13)(i): Anchorages were not capable of supporting at least 5000 pounds (22.2 kN) for each employee attached. On or about February 22, 2019 the employer failed to ensure that the anchorage point for use as part of the personal fall arrest system (PFAS) to protect employees from falling into the G1 Grinding granulator was capable of supporting 5000 pounds of downward force. Employees utilized a PFAS in which the anchorage point was a metal constructed guardrail system.

29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the devices; On or about February 22, 2019 the employer failed to ensure that lockout devices on the E3 Davis Standard Drive identified the employee applying the device on the hazardous energy lockout points. A lockout device was placed on the electrical shutoff of the equipment and secured, however did not identify the employee.
