29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a) Alliance Residential Company at 82 N. Unita Way, Denver, CO: On and before March 6, 2019, the controlling, creating, exposing, and correcting employer did not ensure a safety program was developed and implemented for the work environment, to include regular inspections made by a competent person of the job-site, materials, and equipment. A temporary traffic control zone had been established at Unita Way and E Academy Place. Four temporary employee flaggers had worked in the temporary traffic zone. This condition potentially exposed employees to struck-by hazards from vehicles.  (b) Alliance Residential Company at 82 N. Unita Way, Denver, CO: On and before March 6, 2019, the controlling, creating, and correcting employer did not ensure a safety program was developed and implemented for the work environment, to include regular inspections made by a competent person of the job-site, materials, and equipment. A temporary traffic control zone had been established at Unita Way and E Academy Place. A subcontractor employee, conducting siding work, was tied off to the basket of an aerial lift that was parked in the closed northbound lane on Unita Way in the temporary traffic control zone. This condition potentially exposed the employee to struck-by hazards from the basket or boom, if the aerial list was stuck by a vehicle, and to caught-between hazards between the ground and basket or boom, if the aerial lift was struck by a vehicle and was tipped-over.

Unita Way, Denver, CO: On and before March 6, 2019, the controlling, creating, exposing, and correcting employer did not ensure that barricades were used for the protection of employees that conformed to the portions of the American National Standards Institute D6.1-1971, Manual on Uniform Traffic Control Devices for Streets and Highways, relating to barricades.
