29 CFR 1960.8(a): The head of the Agency did not furnish to each employee employment and a place of employment which was free from recognized hazards that were causing or were likely to cause death or serious physical harm, in that employees were exposed to struck-by and crush hazards:  On or about 02/06/19 and at times prior thereto employees walking/working in proximity of storage racks throughout the warehouse were exposed to be struck-by and/or crushed by metal racks and stored materials in the event of a collapse of the metallic storage racks, with bent, damaged, and/or unmatched structural components from different manufacturing/design sources. The owner did not maintain the structural integrity of the installed rack system by assuring proper operational, housekeeping, and maintenance procedures in order to correct deficiencies including (but not limited to) the following:    a) Warehouse, Building #380 - Portable metallic pallet racks were not tiered in a safe, stable way. The stacked units did not match and did not maintain the vertical direction, affecting the load capacity and stability.    b) Warehouse, Building #380 - The arm of a cantilever rack was pulled upwards. The arm and bolts were damaged and deformed. The system was not unloaded and taken out of service until inspected and repaired.   c) Warehouse, Building #380 - Maximum loads and load distribution guidelines were not followed. A cantilever rack section dedicated to sheet metal storage was subjected to overload. The shelves were deformed, and the rack system was not unloaded and taken out of service until inspected and repaired.   d) Warehouse, Building #380 - Damaged wood pallets were used to support, elevate, and transport loads.                   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
