29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. Meat Box Cooler, Outside Wall. On or about March 11, 2019, equipment was not installed and used in accordance with the instructions included in the listing or labeling. A flexible heating cable was installed on the outside of the metal cooler wall. The cable was exposed to physical damage from rolling metal product carts stored along the outside of the cooler wall. Employees were exposed to electrical shock and burns from arcing and electrical fires. There is no abatement documentation or certification required for this item.

On or about March 11, 2019, a flexible heating cable was run from an electrical box on the outside of the cooler through a hole in the cooler wall. Employees were exposed to electrical shock and burns from arcing and electrical fires. There is no abatement documentation or certification required for this item.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for the fixed wiring of a structure. Meat Box Cooler, Outside Wall. On or about March 11, 2019, an extension cord was connected to the power cord of a condensate pump and plugged into an electrical outlet on the outside of the cooler wall. Both flexible cords were affixed to copper pipes that were attached to the outside cooler wall, as a substitute for fixed wiring. Employees were exposed to fire hazards. There is no abatement documentation or certification required for this item.
