29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. a. On or about March 12, 2019, at the addressed jobsite, the temporary power outlets used to power portable tools and equipment were not ground fault circuit interrupter type receptacles nor did the employer have an assured grounding program for the jobsite, thereby exposing employees to electrical hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about March 12, 2019, at the above addressed jobsite, employees performing residential carpentry operations on a roof, working on a second level with lack of guardrails, and working on the ground floor with no guardrails around the basement access, were not protected from falls, thereby exposing employee to fall hazards. Legendary Construction LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1279864, citation number 1, item number 1 and was affirmed as a final order on January 18, 2018, with respect to a workplace located at Lot 6 Dunmoor Lane, Homer Glen, IL.
