29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards. On or about 3/19/2019, Antonio Yanez DBA Antonio Yanez was installing roofing materials on a roof of a two-story residential apartment building located at 3630 Imperial Gardens Drive in St. Ann, MO. The foreman and four employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems while installing roofing materials on a sloped roof approximately twenty-four feet above the ground. The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate hazards.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On or about 3/19/2019, Antonio Yanez DBA Antonio Yanez was installing roofing materials on a roof of a two-story residential apartment building located at 3630 Imperial Gardens Drive in St. Ann, MO. The foreman and four employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems while working on a sloped roof approximately twenty-four feet above the ground. ANTONIO YANEZ DBA ANTONIO YANEZ was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1313177, citation number 2, item number 1, issued on 5/15/2018 and was affirmed as a final order on 9/26/2018, with respect to a workplace located at 217 Old Ballwin Road, Ballwin, Missouri, 63021.
