29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:  On or about March 14, 2019 employees working at the "Viking" 504, TC 1005A and pallet stacking machine in the Medusa building were exposed to amputation hazards where sprocket and chains were not guarded on the chain conveyor leading to the pallet stacking machine from the "Viking" pallet manufacturing machine.  On March 14, 2019 an employee leaning over this conveyor to reach into the stacker suffered fingertip amputations of the fourth and fifth fingers of his left hand.  PalletOne of Florida, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.219(f)(3) which was contained in OSHA inspection number 1182754, citation number 1, item number 1 and was affirmed as a final order on November 29, 2016, with respect to a workplace located at 566 Baxley Hwy in Hazlehurst, Georgia.
