29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system. a) At the jobsite, on or about 3/20/19: Workers were accessing the roof from the third floor and through a opening in the roof, where a ladder was used to access roof trusses, and where workers would then climb the trusses onto the roof surface.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) At the jobsite, on the roof, on or about 3/20/19: Workers were using personal fall arrest equipment while working on an 8:12 pitch roof with a ground to eave height of 30 feet. The workers deployed their lifelines with enough play that would ensure a greater than 6 foot free-fall in a fall event. PVC Exteriors was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.502(d)(16)(iii), which was contained in OSHA inspection number 982841, citation number 1, item number 1b and was affirmed as a final order on 8/13/14, with respect to a workplace located at 3334 Astilbe Path, Liverpool, NY 13088.
