29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    a) North side of home, front, east end, on or about 4-25-19:  Employees were accessing and exiting a scaffold platform while climbing over the ladder jack brackets, exposing the employees to a fall hazard of 12 feet 2 inches.     b) South side of home, back, east end, on or about 4-25-19:  Employees were accessing and exiting a scaffold platform while climbing over the ladder jack brackets, exposing the employees to a fall hazard of 11 feet 2 inches.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) North side of building, garage area, on or about 4-25-19:  Employees were using a portable fiberglass extension ladder for roof access/egress, with side rails that were 16 inches above the landing surface, exposing the employees to a fall hazard of 12 feet 2 inches to the ground below.
