29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed. The employer is failing to ensure that all employees are protected from fall hazards when utilizing a ladder. This was most recently documented at the jobsite located at 11550 Pawnee Rd., Omaha, Nebraska 68142 on March 22, 2019. Employees were using a twenty-five foot extension ladder to enter and exit the roof. The ladder did not have the side rails extended at least three feet above the landing surface on the edge of the roof. The ladder was extended one foot above the eave. The ground to eave height of the garage measured approximately 15 feet, 2 inches.
