29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems:      The employer is failing to ensure that all employees are protected from fall hazards when performing steel roofing operations. This was most recently documented at the jobsite located at 11550 Pawnee Rd., Omaha, Nebraska 68142 on March 22, 2019. Employees were working on the roof of a new garage structure made of steel without utilizing fall protection and were exposed to falls of over 15 feet, 2 inches to the ground below. The pitch of the roof was 6:12.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed.    The employer is failing to ensure that all employees are protected from fall hazards when utilizing a ladder. This was most recently documented at the jobsite located at 11550 Pawnee Rd., Omaha, Nebraska 68142 on March 22, 2019. Employees were using a twenty-five foot extension ladder to enter and exit the roof. The ladder did not have the side rails extended at least three feet above the landing surface on the edge of the roof. The ladder was extended one foot above the eave. The ground to eave height of the garage measured approximately 15 feet, 2 inches.
