This was most recently documented on or about January 28, 2020 at 1424 East 26th Street, Fremont, NE 68025. Operator of the powered industrial truck had not successfully completed the training required by paragraph (l) of the standard. An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years.
