The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a) On or about 03/22/2019 - 913 Brooke Terrace, Moody, AL, employees installing shingles on a steep-sloped (10:12) roof at approximately 10ft. 5in. feet above the ground were not using the appropriate fall protection.   b) On or about 03/22/2019 - 913 Brooke Terrace, Moody, AL, employees installing shingles on a steep-sloped (10:12) roof at approximately 9ft. 2in. feet above the ground were not using the appropriate fall protection.
