29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Bravo Pack Inc.- Bag machine #1 thru #7: The Kraft paper bubble film laminating bag making machines have multiple pinch points and rotating parts exposed. The machines use two layers of paper and two layers of bubble film attached together with hot melted glue. The machine has heat sealing knives and pull rollers that press against the product to seal it. The heat sealing knives (presses) and pull rollers were unguarded, observed on or about 03/21/2019. b) Bravo Pack Inc. - Stretch film making machine: The machine Model # 3800, S/N BV2J1805 had missing guards between pull cylinders, occurred 03/21/2019.
