29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the forks.      a) Charles Dr., Egg Harbor Twsp., NJ: On or about March 27, 2019, a Silva Brothers Construction Carpentry Inc employee was performing framing activities from a plywood sheet platform supported on the forks of a Lull Rough Terrain Fork Lift.  The platform was not attached in any way to the forks of the RTFL exposing the employee to a fall hazard of 15-20 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) Charles Dr., Egg Harbor Twsp., NJ: On or about March 27, 2019, a Silva Brothers Construction Carpentry Inc employee was performing framing activities from a plywood sheet platform supported on the forks of a Lull Rough Terrain Fork Lift.  The employee was not using any form of fall protection, exposing him to a fall hazard of 15-20 feet.
