Employees engaged in residential framing activities worked from the top step of a fiberglass A-frame ladder and used the top step as a means of access to the scaffold platform. Employees engaged in residential framing activities worked from the outside edges and the top plate of a 5 story building without the use of a guardrail system, safety net system, or personal fall arrest system.   Violation observed on or about November 13, 2019  Silva Brothers Construction Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1138034, citation number 1, item number 1, and was affirmed as a final order on September 7, 2016, with respect to a workplace located at 425 Inman Ave., Building 8, Colonia, NJ 07067
