29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a. On or before March 28, 2019, the employer failed to initiate and maintain a safety and health program such as but not limited to a fall protection program, thereby exposing employees to a 12 foot 3 inch fall hazard.  Sanchez Siding Services was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1356054, citation number 1, item number 1 and was affirmed as a final order on March 4, 2018, with respect to a workplace located at 1288 Sunbury Meadows Drive, Sunbury, OH 43074.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  a. On or before March 28, 2019, the employer failed to train employees on the jobsite hazards or the regulations required to control or eliminate the hazards, thereby exposing employees to a 12 foot 3 inch fall hazard.   Sanchez Siding Services was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1356054, citation number 1, item number 2 and was affirmed as a final order on March 4, 2018, with respect to a workplace located at 1288 Sunbury Meadows Drive, Sunbury, OH 43074.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.   a. On or before March 28, 2019, where an employee was installing siding to the exterior of a house while working from a pump jack scaffold, the employee was not protected from falls, thereby being exposed to a 12 foot 3 inch fall hazard.   Sanchez Siding Services was previously cited for a Repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 1356054, citation number 2, item number 1 and was affirmed as a final order on March 4, 2018, with respect to a workplace located at 1288 Sunbury Meadows Drive, Sunbury, OH 43074.
