29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) 221 Sunset Blvd, Chambersburg PA - On or about March 20, 2019, and times prior thereto, the employer did not develop, document, and implement a procedure for the control of hazardous energy when employees change the tooling on hydraulic press brakes, such as, but not limited to, an Amada HFBO hydraulic press brake.      Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    a) 221 Sunset Blvd, Chambersburg PA - On or about March 20, 2019, and times prior thereto, employees changing the tooling on hydraulic press brakes, such as but not limited to, an Amada HFBO hydraulic press brake, were not adequately trained on the safe application, usage and removal of energy control devices.    Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a) 221 Sunset Blvd, Chambersburg PA - On or about March 20, 2019, an employee's fingers were crushed by an Amada HFBO hydraulic press brake. The press brake was not shut down, energy isolation devices were not applied to control energy sources, and verification of isolation of all energy sources was not performed prior to changing the tooling.    Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      a) 221 Sunset Blvd, Chambersburg PA - On or about March 20, 2019, the employer failed to ensure that one or more methods of machine guarding, such as a barrier guard, was provided to protect employees from struck-by and crushing hazards created by inadvertent contact with the point of operation of an Amada HFBO hydraulic press brake when walking in the aisle-way behind the press and when accessing the area behind the press to retrieve stored materials.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
