A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a. Located between lot 6 and lot 7 near 3889 Shannon Drive in Canal Winchester, Ohio: On or about April 4, 2019, an employee was performing pipe laying work in an excavated trench that had a trench box. The employee was not provided with a safe means of egress to and from the trench box, thereby exposing the employee to a crushed by hazard while accessing to and from the trench box.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a. Located between lot 6 and lot 7 near 3889 Shannon Drive in Canal Winchester, Ohio: On or about April 4, 2019, an employee was performing pipe laying work in an excavated trench that was approximately fifteen foot in depth. An eight foot deep trench box backfilled approximately four foot in combination with sloped trench walls did not provide adequate protection for the employee working in the trench, thereby exposing the employee to a crushed by hazard. b. Located between lot 6 and lot 7 near 3889 Shannon Drive in Canal Winchester, Ohio: On or about April 4, 2019, an employee was performing pipe laying work in an excavated trench that was approximately fifteen foot in depth. The employee was not protected from cave in hazards while accessing to and from the trench box, thereby exposing the employee to a crushed by hazard. Savko & Sons, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1059864, citation number 1, item number 2 and was affirmed as a final order on July 1, 2015 with respect to a workplace located at Main Street/Keswick in New Albany, Ohio.

29 CFR 1926.651(e): Exposure to falling loads. No employee shall be permitted underneath loads handled by lifting or digging equipment. Employees shall be required to stand away from any vehicle being loaded or unloaded to avoid being struck by any spillage or falling materials. Operators may remain in the cabs of vehicles being loaded or unloaded when the vehicles are equipped, in accordance with 1926.601(b)(6), to provide adequate protection for the operator during loading and unloading operations. a. Located between lot 6 and lot 7 near 3889 Shannon Drive in Canal Winchester, Ohio: On or about April 4, 2019, an employee working in a trench box was exposed to a struck by hazard while working under the excavator bucket that was loaded with gravel.
