29 CFR 1910.28(b)(1)(i)(A): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system.  a) Mold Room/Disa Department, Pour Deck - On or about 4/10/19, the employer exposed employees to the fall hazards in that barriers were not provided to prevent employees from falling or stepping into the tough when manually pouring molten metal from the ladle and while taking samples for quality control.
